Derek Atkins 4e43ae10f3 Preparation for QofEntity -> QofInstance: ...

- Convert (QofEntity*)x to QOF_ENTITY(x)
- Move qof_entity_init() and friends into qof_instance and rename
- Move QofEntity contents into QofInstance
(note:  this doesn't compile)
