runtime(rmd) Update ftplugin and syntax files (vim/vim#13193)

ftplugin/rmd.vim:

  - Set 'commentstring' dynamically according to code region.

syntax/rmd.vim:

  - Include syntax highlighting of fenced languages dynamically.
  - Add conceal char for line break.
