Reduce ALL_FLOWS lock contention

Hide the live flow table behind accessors, publish an ArcSwap active-flow snapshot for informational readers, and move export/BPF cleanup work outside the ALL_FLOWS lock.
Preserve existing wire and circuit identity behavior while adding regression coverage for snapshot readers, stale filtering, exact-rate serialization, and QoO/RTT thresholds.
