There is already a generic one with the Grid implementation
being the template parameter in the header file. The other
implementation using UnstructuredGrid does exactly the same.
Therefore we remove it with this commit.
Previously, the eclipseGrid used by EclipseWriter was constructed from
the one in the EclipseState with the current CpGrid. Unfortunately the
latter was the distributed version resembling only the local part that
the processor works on. Therefore the information about the active cells
was wrong when writing results (which raised an exception in the writer).
With this commit we construct the EclipseWriter before distributing the grid
and use this writer later on in the OutputWriter.
All ranks were still writing to step_timing.txt at the same time.
This made it unusable for parallel runs. With this commit only
one processes writes to this file.
With GCC version (Debian 4.9.2-10) 4.9.2 we get the following error
when compiling with -std=c++11 (default for dune 2.4):

Check that we actually have data values for relative permeability
properties {WAT,OIL,GAS}KR before attempting to output the arrays.
While here, also correct an apparent misprint in the criterion for
whether or not to activate relperm output. We should check
'liquid_active' and 'vapour_active', not 'aqua_active', when
considering OILKR and GASKR properties respectively.
Previously, the call was made after the grid was distributed.
This means that each process wrote it, but only with his cells
active which was just a part of the whole domain.
With this commit we make the writeInit call before distributing the
grid and make sure that only one process calls it.
At the moment, for the ParallelDebugOutput, we put a dummy
dyanmic_list_econ_limited, not sure how it will the parallel running.
The basic problem is that when initialzing the globalWellState_, what
will happen if they can not find state information for a well in the Wells*.
If some defaulted values are used, then no big problem here.
Changes to BlackoilOutputWriter as mandated by the split and rewrite of
opm-output. Notable changes:
* BlackoilOutputWriter is no longer a child class of OutputWriter.
* Minor interface changes; writeTimeStep requires a Wells pointer
* restore requires a Wells* pointer
* VTK/Matlab support rewrites; no longer inherits OutputWriter
* WellStateFullyImplicitBlackoil::report added, to write its data to a
opm-output understood format
Relies on utility/Compat.hpp for quick conversion to the opm-output
defined formats.
The non-cartesian connections are required by the output facilities,
while the truly non-neighbour connections (meaning those not in the grid)
should be used by all other code.
