Cleanup in ngx_event_quic.c:
    + reorderded functions, structures
    + added missing prototypes
    + added separate handlers for each frame type
    + numerous indentation/comments/TODO fixes
    + removed non-implemented qc->state and corresponding enum;
        this requires deep thinking, stub was unused.
    + streams inside quic connection are now in own structure

...
