Adds ``smartquotes``, ``smartquotes_action``, ``smartquotes_excludes``
configuration variables.

- if ``smartquotes`` is set to False, then Smart Quotes transform is not
  applied even if a Docutils configuration file activates it,

- the current default of ``smartquotes_excludes`` deactivates Smart
  Quotes for Japanese language, and also for the ``man`` and ``text``
  builders.

  However, currently ``make text html`` deactivates Smart Quotes for
  ``html`` too, and ``make html text`` activates them for ``text`` too,
  because the picked environment is shared and already transformed.

- now Smart Quotes get applied also when source documents are in
  Markdown or other formats.

jfbu 5331ab2ef9 Fix #3851 and fix #3706 about box drawing characters for PDF output 
Do not tex-escape them, but provide suitable definitions for pdflatex
only (and platex). Define the 4 box drawing characters needed by unix
tree command with charset utf8. Also define the U+2572 as it was
previously escaped for all engines.

No definitions for xelatex/lualatex as it is then only a matter of using
a font providing them (a priori, only the mono font needs definition, as
the characters will be used in literal blocks).

jfbu 0b0bbfcb5b Backport docutils.utils.smartquotes fixes as of r8097 
This incorporates fixes from 0.14rc1 which were not in the PR #3666
merged in 1.6.1, and extra language configurations and fixes for quotes.

Only active if Docutils is at 0.13.1 or earlier.

jfbu 7165a4f1e4 Fix #3788: Docutils emits warnings for unsupported languages 
This monkey-patches docutils.languages.get_language() to suppress
Docutils emitted warning when the document settings use a language_code
for which Docutils has no ready-made localisation. The language_code is
needed for functioning of the smartquotes transform.

jfbu 6422f7b8c9 Fix #3742: missing `Index` bookmark and TOC entry in PDF builds at RTD 
The trick is to create an Index TOC/bookmarks entry in anticipation of
makeindex execution, as RTD (currently) does only one pdflatex run after
makeindex.

The workaround works for Sphinx own docs, which uses ``'manual'``
docclass and puts the general index at very end of document.

jfbu ae1455077b Fix #3746: PDF builds fail with latexmk version 4.48 or earlier 
This is tested to work with latexmk version: 4.39 (10 Nov 2013).

For xelatex engine and with latexmk 4.52b or later (Jan 2017), setting
LATEXMKOPTS Makefile variable on command line to "-xelatex" results in
faster builds, because PDF is produced only on last run.
