Georg Brandl 13f6fc7994 * `apidoc now ignores "_private" modules by default, and has an option -P` 
to include them.

* ``apidoc`` now has an option to not generate headings for packages and
modules, for the case that the module docstring already includes a reST
heading.

Also add a workaround on the consumer side in intersphinx, so that links
